The photo is probably from a dermatological or skincare procedure—more precisely, a comedone or blackhead extraction. During this kind of treatment, dead skin cells, sebum (oil), and other debris are forced out of clogged pores using an instrument called a comedone extractor. The tiny pimples in the picture are probably open comedones (blackheads) or closed comedones (whiteheads), which are frequent types of acne.
The normal irritation brought on by the extraction process may be the source of the redness and small swelling surrounding the pores; however, this irritation usually goes away subsequently.
